Quarterly attestation form

New question added to attestation form required for in-person learning 

On Jan. 25, the state Department of Health updated their K-12 Schools Guidance to include an additional question that families should review on a daily basis to help determine if they can send their child to school.

The new question: Has your child had a positive COVID-19 test for active virus in the past 10 days, or are you awaiting results of a COVID-19 test?

This makes up now five questions on the District’s attestation form, which must be signed each quarter in order for your child to enter the classroom. While completion of the form is quarterly, families need to ask the five questions on a daily basis before they send their child to school for in-person learning.

If your child will be attending school in person, please complete the attestation form through Family Access or print and sign a hard copy to bring to your school (English | Spanish). Scanned copies and electronic signatures are not allowed. If you have already completed an attestation form, you do not need to sign the updated version.

With our expanded return to in-person learning, health and safety is our top priority. Families have an important role in helping ensure the safest environment possible for students and staff.

It’s critical that all families with students in the In-Person Hybrid Learning model complete an attestation form for each child once every quarter. Completion of this form is required before entering the school building. The preferred and most efficient method to fill out the form is through Family Access. You may also download and print a hard copy of the form (English) (Spanish) and bring it to the school. Forms are not needed for students in Full-Time Distance Learning.

The quarterly attestation form affirms that the parent/guardian will check their child daily for all symptoms of COVID-19 (symptoms listed on form) and agree not to send their child to school if the child has any symptoms, has been in close contact with someone with COVID-19, or has been told to self-monitor, quarantine or isolate. They also attest to report any signs of illness or contact with someone who has symptoms of COVID-19 to the school.

Please complete this form in Family Access prior to your child’s first day of in-person learning. If you have questions or need assistance completing the form, please call your school.
