- You will find the Tort claim form below. These instructions and the Tort claim form is available on the Sumner Bonney Lake School District website or by contacting the Legal Affairs Department located in the Administrative Services Building.
- Tort claim form must be typed or printed clearly in ink.
- If the requested information cannot be supplied in the space provided, please use additional blank sheets so your claim can be easily read and understood.
- Provide all requested information and any available documents supporting your claim.
- The authorized party must sign the form and have it notarized.
PRESENTATION
Presentation or delivery of the the properly completed and signed Tort claim may occur via:
A. Personal delivery, during the business hours of 7:30 a.m. – 4:00 p.m., Monday through Friday, except holidays.
B. Registered mail.
C. Certified mail (with return receipt) to registered agent.
